Die beteiligten Spit\u00e4ler und Kliniken m\u00fcssen die wirtschaftliche Effizienz und Effektivit\u00e4t ihrer Pflegeleistungen nach standardisierten Tarifmodellen und definierten Qualit\u00e4tsregeln (ANQ) darlegen.<\/p>\n<p>In diesem Zusammenhang hat der Verband PLATEFORME-REHA im Jahr 2017 das Projekt &#8222;Patientenpfade&#8220; ins Leben gerufen, mit dem Ziel, einen &#8222;betriebswirtschaftlichen&#8220; Bezugsrahmen f\u00fcr die Verbandsmitglieder zu schaffen, der den Prozess der Patientenbetreuung und die im Rahmen eines station\u00e4ren Rehabilitationsaufenthalts zu erbringenden Leistungen abdeckt.<\/p>\n<p>Das Ergebnis dieser ersten Projektphase war ein Bericht mit Empfehlungen f\u00fcr das Patientenmanagement und die medizinisch-therapeutischen Pflegeleistungen in der station\u00e4ren Rehabilitation in der Schweiz. Diese Empfehlungen ber\u00fccksichtigen die nationalen Standards (DefReha) und erm\u00f6glichen den Mitgliedern, die bevorstehende Einf\u00fchrung des neuen Tarifmodells zu erf\u00fcllen. Sie erm\u00f6glicht auch den Kostentr\u00e4gern (Kantone und Versicherer) einen informierten Blick auf die Arbeit und den Einsatz f\u00fcr die Patienten in den Mitgliedsspit\u00e4lern und -kliniken von PLATEFORME-REHA.CH.<\/p>\n<p>Der Abschlussbericht zu diesem Projekt ist im Mitgliederbereich verf\u00fcgbar.<br \/>\nIn einer zweiten Phase bietet PLATEFORME-REHA.CH seinen Mitgliedern Unterst\u00fctzung an, insbesondere durch die Nutzung von zwei Referenzinstitutionen als Ressourcen, um ihren Rehabilitationsprozess zu optimieren.<\/p>\n<p>Das Hauptziel ist die Harmonisierung der Kommunikationsmittel und die Etablierung einer gemeinsamen Sprache bei Bewertungen und Kolloquien, um den interdisziplin\u00e4ren Austausch zu vereinfachen. Juni 2019 einen Round Table organisiert. Das Treffen war ein Erfolg und PLATEFORME-REHA.CH lancierte die 2. Phase des Projekts, in deren Folge eine Nomenklatur f\u00fcr die\u00a0 Rehabilitationspflege ver\u00f6ffentlicht wurde.<\/p>\n<p>Gleichzeitig wurde die Bezeichnung \u00ab ABTP \u00bb\u00a0 durch die Bezeichnung \u00ab Rehabilitationspflege \u00bb ersetzt um Verwechslungen zu vermeiden.<\/p>\n<p>Im Jahr 2024 hat PLATEFORME-REHA.CH die dritte Phase lanciert, um Empfehlungen f\u00fcr die Dauer in Minuten der Rehapflege und ihrer \u00c4quivalente in der Grundpflege vorzugeschlagen. Dieser Abschlussbericht wurde am 7. Wir arbeiten mehreren Partnern kontinuierlich an Schulungsprogrammen, um den Anforderungen im station\u00e4ren Rehabilitationsbreich mit aktuellen Kompetenzen gerecht zu werden. Eine Task Force, welcher Schweizer Ausbildungsinstanzen sowie zwei Vorstandsmitglieder unseres Verbands angeh\u00f6ren, koordiniert und beaufsichtigt die Organisation und die Weiterentwicklung des Schulungsprogramms basiert au den Bed\u00fcrfnissen der Praxis. Sie ist direkt von den erbrachten Leistungen abh\u00e4ngig und erm\u00f6glicht daher, dass die Tarife die damit verbundenen Kosten widerspiegeln.<\/p>\n<p>Diese Tarifstruktur funktioniert nach einem \u00e4hnlichen Modus wie die SwissDRGs, mit dem Unterschied, dass keine Fallpauschalen, sondern Tagespauschalen generiert werden. Die F\u00e4lle werden nach dem Erreichen vordefinierter Kriterien gruppiert und folgen einer Entscheidungsbaumlogik. Die Daten aus der medizinischen Kodierung (ICD 10 und CHOP) bestimmen eine Einstufung in &#8222;readaptation cost groups&#8220; (RCG).<\/p>\n<p>Diese neue Tarifstruktur wurde offiziell per 1.
